How to Set Up ODK Collect

1. Install ODK Collect on your Android device
2. Open ODK Collect and tap the menu button (3 dots)
3. Select Settings → Server
4. Enter the server URL: https://afrosurvey.com
5. Enter your Afro Survey username and password
6. Go back to the main menu and tap Get Blank Form
7. Select this form to download it to your device
